At D&M Fernandez Dental, completing patient forms in advance helps start your visit on time and personalize your care. Fill out medical history, insurance details, and consent forms before your appointment. Please bring a current medication list and any recent dental x-rays. New patients should arrive 10 minutes early to review information and verify identification and insurance. If you prefer to complete forms in the office, plan a few extra minutes and bring your insurance card.
